I suspect a fake! He might not be a real doctor. He started out well lubricating the sore area (which did not really look swollen too badly.). But the manipulations caused MORE swelling. I would have inserted a lubed finger to massage the internal areas. Attempting to drain prostate liquids to reduce pain. The doctor did not seem to be focused on the patient's relief. He also did not explain turning him over to initiate a counter sensation to to sore ass.
